Revolutionizing care by connecting physicians to local pharmacies to provide telehealth services for diabetic patient management

Perfect Balance Healthcare (PBH), established in 2017, acts as the liaison between patients, their caretakers, and insurance providers to seal the gaps in their caregiving patients control over their health through medication education and optimization and cost-reduction solutions. Dr. Nandita Koodie empowers patients to establish permanent success in achieving their health goals through guidance, communication with physicians and focus on nutrition and lifestyle modifications.
 Perfect Balance Healthcare has established Pharmacist and Physician groups on Facebook and LinkedIn to encourage collaborative, coordinated community care. Dr. Koodie now actively speaks through all media outlets and in her community to inspire others to serve as the glue between patients and their providers. She promotes the connection of physicians to their local pharmacies through billable telehealth services that establish a continuum of care from their doctors’ offices to patient homes bringing healthcare back to the basics.

PBH is shifting the focus of care from the heavily incentivized smallest subset of elderly patients to proactive preventative care of patients between the ages of 30-50 who make up the largest population of Americans suffering from chronic diseases.
34.2 million people, or 10.5% of the U.S. population, have diabetes.
Through their Diabetes Management Telehealth Program (DMTP™), PBH has provided a model that allows pharmacists to work dually in physician’s offices and their local independent pharmacy as well as remotely to guide diabetic patients off their medications. The clinical pharmacist utilizes continuous glucose monitoring biotechnology to guide patients through micro changes in their life habits.
Additionally, the pharmacist provides ongoing clinical support to both practice sites easing the burden of patient care and increase revenue allowing them to accommodate more patients in need. Patients enter a community of over a million people who have found success or are sharing in their transformation alongside life coaches equipped with the tools to guide them. DMTP is composed of physician and pharmacist designed components, is self-funded and is backed by over a decade of clinical evidence to support positive patient outcomes.
Dr. Koodie is working with caretakers to provide them with to the tools necessary for successful program operation through practice site integration and web-based software solutions. Telehealth services allow for the construction of a network health hub that functions to connect the dots for patients who are struggling to get their providers all on the same page.
 Right now, there is a shortage of physicians and an overabundance of pharmacists, medication therapy experts, ready and more than willing to lend assistance in guiding patients they already share beyond diagnosis to achieve treatment success through holistic care.
